Registration with the practice is restricted to patients who are moving into our practice boundary as noted on the map below. To register, you need to complete the online registration or a registration form obtained from our reception desk. Please be thorough and complete all sections of the registration form, as unfortunately we are unable to process incomplete forms, and this may delay your registration. We aim to process registration applications within five working days.
Please be aware a member of our team might contact you for further information after you submit your registration online and we might ask you to come in to the surgery to provide further information.
In order to establish NHS entitlement we need to see you in person, with two forms of identification, before you can be registered. One piece must be photo ID (passport, drivers license, identity card) and another showing your name and address (utility bill, rental agreement). It is the applicant’s responsibility to prove their entitlement to NHS Services when applying to register. Where there is any doubt, for example, if you were born or have lived abroad for a considerable period, the staff may ask for further documentation such as a passport, valid visa, medical card or evidence of permanent or regular return to the UK, if this has not already been presented. This process follows NHS Lothian and NHS Scotland guidelines and we would be grateful for your cooperation in this routine administrative matter.

Please note we are unable to register patients who are outside of our catchment area or currently registered with a GP surgery what covers the catchment area where you live.
